Co-payments are expected at the time of service unless you have made payment arrangements with our billing office (513-985-3700) prior to your appointment. For your convenience, we accept Mastercard and Visa.

Please know the details of your insurance policy. It is your responsibility to make sure that we are in the approved provider panel for your insurance company so that you do not incur any additional out-of-pocket expense. Prior approval is frequently needed for surgery, MRI testing or physical therapy. Have your insurance information and card available during your visit and be sure to inform our receptionist of any changes in your insurance coverage.
